WHAT TO SUBMIT Once you have maintained the Model Classroom criteria, submit the following documents to Renaissance Learning. Reading Model Classroom Application. Reading Classroom Questionnaire. Reading Classroom Checklist. Qualifying 12-week Diagnostic Report. STAR Reading Summary Report or STAR Early Literacy Summary Report from the first STAR test of the year. (If your school does not use STAR Reading software, please submit Standardized Test Summary Chart or Alternate Assessment Checklist.)
