Recertification Master Classroom To recertify for Model Classroom you must maintain the same criteria as listed for Model Classroom for any 12-week period. The following documents must be submitted when recertifying for Model Classroom status: 1. Recertification Update Form - Classroom or School. 2. Qualifying 12-week Diagnostic Report.
